Help with Children in Need Expression of Interest Form

Hi, can anyone help with the Children in Need Expression of Interest Form please?
It says to use the drop-down choices of true and false for the answers, but I cannot see the drop-down option. I've tried closing it down and reopening in another browser and another programme, but neither work.
Can anyone help please?
