Looking for pointers on 360 Appraisal resources

Hi
I'm looking to do a 360 Appraisal with a senior staff member that I have only known for a couple of months and will be leaving in the next 6 months.
Can anyone point me in the right direction as to where I can find a free resource for such an appraisal or something similar whereby my staff member could undertake most of the appraisal themselves - self-assessing, from which I then moderate/listen/reflect and provide a few pieces of advice.
