What to include in exit surveys?

Hi everyone! thanks for adding me to this network and I am pleased to be part of this community. 
I work in the HR team of a charity and I am currently looking to redesign our exit survey that we send out to staff leavers to complete. 
I believe we can get some valuable insights from these exit interviews/surveys and it is important to ask the right questions to get these insights  to make improvements where required. 
We currently have an exit survey but it is really long and wordy and is failing to provide us with good qualitative and quantitative data. 
Hence my question to you all - what is it that you have in place for exiting staff? What are some of the questions that you ask which help you in picking trends? Are you happy to share what you use with me to get ideas on improvements?
