Approving and Signing-off of Last Trustee Meeting Minutes

CC48 states that, at the beginning of a trustee meeting, the trustees should approve the last-meeting minutes. The chair should then sign the minutes and store them safely. 
I have a few questions on this:
1) Does the "chair" refer to the charity chairperson, or the chair of the current meeting? 
2) Is it possible for the secretary instead of the chair to sign the minutes and store them safely? 
3) Do the last-meeting minutes also only need to be approved by those attending the current meeting, or those attednding the last meeting? Do those who approve need to constitute a quorum of the overall Board?
