Which forms do new trustees need to sign? (Anonymous post 🤫)

We are a small advice charity (six employees/ £150k turnover per year) that is looking to review its sickness absence policy.
I am keen to know how much paid/half pay sick leave other charities set so we can estimate whether we need to amend our allowances.
Currently we have the following for any rolling 12 month period:
Probationary period (3 months) = zero
3 mths to 2 years = 10 days full pay
2 years to 6 years = 15 days full pay + 15 days half pay
+6 years = 25 days full day + 25 days half pay.
