Suggestions for Flexible Benefits providers - medcial/life/EAP/Perks

I work for a charity with 150-200 staff. Our benefits are currently pretty basic (pension), eye test vouchers. We want to look at introducing a benefits package but not too costly, does anyone have any recommendations?

We are looking at medical/life cover or medical cashback scheme (ideally a flexible offering where the employee can choose which is most appropriate for them)

We also want to offer an EAP - preferably one that offers a manager's helpline in dealing with difficult conversations with staff, eg managing conflict at work as well as the more traditional employee counselling.

Can anyone also recommend any good providers on discount schemes eg Perkbox. we are already able to register for the Blue Light scheme, so this is less priority.
