Any trustee contracts or governing documents please?

So recently we've had some issues with long standing trustees, one who hasn't done their role in 7 years, been absorbed by the manager. Another almost got Ofsted called on us over a callous decision they were clearly told not to do.

We need some kind of 'contract' stipulating code of conduct, getting DBs in process within 6 months, length of service. Does anyone have anything similar please?
