Appointing a new trustee for CIO

Hi, We a small charity, Sambhali UK, with three trustees, but I am the only active one. I have been diagnosed with terminal cancer and the other two trustees are not able to take on the financial aspects of the running of the charity. However, I do have someone who has volunteered to do that and therefore needs to be appointed as a new trustee. we are a charitable Incorporated organisation. Please could somebody explain how I go about this process and the relevant forms that I will need to complete on behalf of the appointment if you could send copies of those to me I would be extremely grateful as well.
