We are a domestic abuse charity in the UK

We have been contacted by a donor who is planning to make a donation.( money ). She asked us if we could get a copy of a deed of gift confirmation to her - so that there was evidence in terms of tax  for her/her company's own tax affairs , that she had made a gift. I am not sure if she is doing it through a limited company or as an individual .

Does  anyone know where I can get a template? where I can get more info as to a deed of gift confirmation.

I am sure that she is not talking about gift aid , this is for her to show she made a gift .
