How do I update an outdated charity governing document?

Hi,

I have recently joined a tiny charitable trust as Treasurer and soon to be trustee. However the governing document that sits with the charity commission is really outdated and requires additional trustees from local organisations that no longer exist.

I could do with some help on updating this. I think the first place to start is getting hold of some generic forms for the appointment and retirement of trustees. Could anyone point me in the right direction please?
