Rebranding our charity profile. What are the pitfalls?

Hi - we are a small children's charity (turnover circa £180,000 per annum) & have been operating for 13 years. During that time we have developed & expanded our work & range of support for local residents, families & children.
We feel that our name doesn't now reflect how diverse our work is so are thinking about a re-brand in the coming 12 months. Does anyone have experience of going through this (probably tortuous) process?
