Facilitator needed 'Organisational Structure'—do any of you work 'without' a formal line management structure?

Hello there,
I work for a small charity in Hackney. My team want to run an online workshop on 'organisational structure' led by someone who's had first hand experience of working for a third sector/community organisation that does not use traditional line management hierarchy and is more progressive and thinks 'out of the box' etc. 
We have a flat line management structure presently. A core team of 9 staff without any 'managers'. It's a fantastic concept but it's leading to uncertainty over who is accountable for big decisions and how to ensure we have a structured way to give and receive performance feedback, welfare and appraisals. 
We'd like a workshop to cover issues like;
  • Day To Day Working Culture 
  • How Decisions Are Made 
  • Roles and how they support each other, work together and overlap - how staff can feel supported in the absence of a formal line management structure
  • Legal Requirements 
Any recommendations on someone to deliver such a workshop would be great.
