Difficulties recruiting for volunteer coordinator

Hi everyone, we're looking to introduce an EDI survey to find out how people are feeling about inclusion, belonging and representation at our charity.
I've developed similar surveys in the past, but I'm keen to benchmark this work against other charity's approaches. Does anyone have examples of similar surveys you would be willing to share? Thanks in advance!
